What to Expect on Your First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

If you are a new patient, please arrive 15 minutes prior to your visit to fill out the necessary forms.  You may also download the forms from this website to be filled out prior to your appointment.

The following forms will be needed to be complete prior to or at your visit: Insurance information, patient privacy policy, consent for photography, a copy of the financial policy and a copy of the cancellation policy signed. You will also be asked to fill out a health history form, which your healthcare provider will review.

Please assist us by providing the following information at the time of your consultation:

Insurance card to be photocopied.

Photo ID- to be photocopied for your protection against insurance card theft.

Your referral slip and any current X-rays or lab work, if applicable .

A list of medications, vitamins and herbal supplements you are presently taking.

 

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
